Terms and conditions of use of the Patient's Private Area
Consultorio Dexeus SAP, a company with registered offices at Gran Via Carles III, 71-75, 08028 Barcelona, Spain (hereinafter, “Dexeus”) provides access to and use of what is known as the “Patient’s Private Area” (hereinafter, the “PPA”) on the website https://evp.dexeus.com under the following terms and conditions of use:
1. Acceptance and modification of the conditions
By accessing and using the PPA you unreservedly accept these terms and conditions of use and, when applicable, subsequent versions hereof. Dexeus may change these terms and conditions of use at any time, for legal, technical or any other reason. The following terms of use will be accessible at any time on this page in their latest version, which will have a date of validation and will have immediate effect from the date of their publication in replacement of the previous ones. It will be understood that users accept the new version of the terms and conditions of use if they continue to access and use the PPA one (1) month after their date of publication.
2. Registration
Only natural persons over the age of 18 years who are legally capable of entering into binding contracts may register as users of the PPA.
3. Data protection
The website www.dexeus.com is owned by Consultorio Dexeus, S.A.P.
The purpose of this privacy policy is to provide information on how Consultorio Dexeus treats your personal data in accordance with current regulations on the protection of personal data.
Data controller
The data controller is Consultorio Dexeus S.A.P, with registered office in Gran Via Carles III 71-75, 08028 Barcelona and VAT number A-08.276.867.
For the treatment of patients and staff selection, Grupo Dexeus Mujer acts as data controller. The entities that make up the co-responsibility of personal data are:

Is it mandatory to provide all the data?
When the data are requested to process the desired action, the necessary nature of the data considered essential shall be informed. If the mandatory data are not provided, it will not be possible to process the request.
How to exercise your data protection rights?
If the processing of your personal data is based on consent, you can revoke the authorisation at any time by writing to the data protection officer at dpd@dexeus.com.
You can exercise your rights of access, rectification, deletion, opposition, portability and limitation by writing to the data controller Consultorio Dexeus SAP, based in Gran Via Carles III 71-75, 08028 Barcelona and VAT number A-08.276.867 or by contacting the data protection officer at the email dpd@dexeus.com. In any case, a photocopy of your identity card or equivalent must be attached. Once the request has been submitted, and the applicant has been identified, he or she will be informed of the proceedings arising from the request within one month. In the case of particularly complex applications, the period may be extended to a further two months (up to a maximum of 3 months), always informing of the extension of the period within the first month. You are also informed of your right, if you do not agree with the processing carried out by our Entity or consider that your rights have been infringed, to lodge a complaint at any time with the competent data protection Authority.
4. Accuracy of the data
You hereby guarantee that the information provided by you during registration is accurate, true and refers to you personally, and that you will keep it updated at all times. Use of data or information that belongs to third parties is not permitted under any circumstances.
5. Access to the PPA
You may access and use the sections of the PPA, such as the medical file, documentation, medical team, appointments and personal data update, only after you have satisfactorily completed registration for the PPA.
6. Personal use
Use of the PPA is personal and non-transferable. You agree to take the necessary precautions to prevent third parties from accessing your personal space in the PPA.
7. Diligent use
You undertake to make responsible use of the PPA in accordance with the law, public decency and the requirements of good faith, exercising due diligence in this respect.
8. Responsibility
You shall not use the PPA in any way that might prevent, damage or impede the normal operation of the PPA or the assets, rights and interests of Dexeus, other PPA users or, generally, any third parties, and you will be personally liable for any loss or damage that you may cause.
9. Cancellation of PPA registration
You may cancel your registration for the PPA at any time by sending an email from the email address that you used to register to Patient Care Services (international@dexeus.com).
10. Incidents and Enquiries
For incidents or enquiries in relation to the use, access or operation of the PPA, please contact us by phoning +34 93 227 48 96 (Monday to Friday, excluding public holidays in Spain, from 9:00-14:00 and from 15:00-20:00 CET) and/or the email address international@dexeus.com.
11. Fraudulent registration, access or use
It is strictly prohibited to register and use the PPA using false and/or stolen identities; in such cases, Dexeus reserves the right to take legal action to enforce the civil and criminal liability arising from fraudulent registration, access or use.
12. Dexeus’ rights
Dexeus reserves the following rights: (i) to carry out any kind of inspections and random checks to verify the identity of users and, when applicable, detect fraudulent registration, access or use of the PPA and/or that which is contrary to these terms and conditions; and (ii) monitor, change, block and/or end access to, operation or use of the PPA, temporarily (for example, for security reasons) or permanently (for example, for breach of these terms of use) for any reason (including Dexeus’s unilateral decision to cease operation of the PPA; in this case all users of this unilateral decision will be informed) at any time and without prior notice.
13. Governing law
These terms and conditions of use shall be governed by the laws of Spain.
14. Jurisdiction and venue
The Courts of the city of Barcelona (Spain) shall have exclusive jurisdiction to resolve any dispute that may arise in connection with the use of the PPA.
